Record a risk assessment you have carried out yourself, in a layout that captures the part people forget: who is doing the further action, by when, and when it gets reviewed.

A free printable manual handling assessment — the load, the task, the place and the person, with what will change recorded against each problem found.
One page. Choose your paper size — both versions are laid out identically, so neither one gets shrunk to fit.
Print it and write on it
Fill it in type and save
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Assess one lifting or carrying task properly, including the question people skip: whether it has to be moved by hand at all.
Warehouses, care services, kitchens, removals, small builders, shops and anyone whose staff lift things regularly.
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
There is no single answer, which is why no figure is printed here. Guideline numbers differ by country, drop sharply depending on how and where the load is held, and are not a safe limit for any particular person. Assess the task.

Record what protective equipment was issued to whom and when it was replaced, with the instruction and fit-check noted at the point of issue.
Capture an incident properly on the day, in the affected person's own words, with the witness details and equipment notes that become impossible to gather later.
