What this sheet is for
Tie an inspection record to a specific piece of equipment by serial number, and record what was found and what was done about it.
Who it is for
Anyone using fall arrest equipment — roofers, steel erectors, arborists, riggers, window cleaners and the people responsible for their kit.
How to use it
- Record the serial number and date of manufacture from the label before anything else.
- Take anything failed out of service immediately and quarantine it, rather than leaving it in the van.
- Record who inspected it and what makes them competent to do so.
- File the sheet for as long as your own rules require.
What is on the sheet
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Questions
Why are no pass or fail criteria printed?
Because they are set by the manufacturer for that product, and printing generic ones would be worse than printing none. Detailed inspection also usually has to be done by a competent person, and how often differs by country and by workplace. Check what applies and record the result here.
