What this sheet is for

Put the survey, the load and the sign-off on one sheet the crew carries, so what was agreed, what was loaded and what was delivered all sit side by side.

Who it is for

Removal firms of any size, from a single van to a crew of four, and man and van operators taking on full house moves.

How to use it

  1. Fill in both addresses and their access notes at survey — floor, lift, parking.
  2. Record the crew, vehicles and the price agreed before the day.
  3. List large items and carton groups as they are loaded, noting existing damage.
  4. Tick each line as it is delivered.
  5. Have the customer and crew leader sign at the far end, with any shortfalls written in.

Questions

Why is there no volume or price table?

Because how you estimate volume and what you charge for it are your own methods. The sheet has a blank for the estimated volume and the price you agreed, and nothing that suggests what either ought to be.

Where do the cartons go?

Group them here by room or by count, and use the Packing and Carton Count Sheet for the number-by-number tally.
