Lash and brow technicians, beauty therapists offering tinting, lifts and extensions, and salons that want one card per client for eye work.
One page, top to bottom:
Printed under the title: One record per client for lash and brow work — the patch test, the eye and skin questions, then every tint, lift, lamination or set of extensions with the products and lot numbers used.
The manufacturer's timings, your record of them. When to patch test, how far ahead, what adhesive or tint to use and how long to leave it are set by the product manufacturer, your training, your insurer and the rules where you work. The columns for those are blank so that you write in what you actually did.
Write-on lines: Client · Contact no. · Date of birth · Technician · Eye or skin conditions, contact lenses, recent eye surgery, medication or allergies declared · Declared on · Patch test — product(s) · Date done · Result · Consent signed on · Retest due (your rule) · Natural lashes and brows — length, density, condition, gaps · Look wanted (natural, full, lifted…)
Table, 8 rows: Date · Treatment · Products & lot nos. · Shade, curl, length · Timing · By · Reaction / result · Next visit
Because the timing comes from the product manufacturer's instructions and your own training, and it changes between products. The column records what you did so the next visit can repeat or adjust it.
Eye work has its own questions — contact lenses, eye surgery, the natural lash condition — and its own product details like adhesive, curl and length. A general card has no room for them; this one is built around them.
Have the product, lot number and skin reaction from last time in front of you before you start this time, on one card per client.
Ask every question before the first cut or colour, write the answers down, and have the client sign what was agreed.
Match last time's colour exactly, or change it on purpose, because the formula and the time it was left are written down rather than remembered.
Keep one card per customer so the history of what was done, what it cost and when the next visit is due lives in one place.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
