Catch chipping, peeling and rubbing on paint that may contain lead before it turns to dust. Each surface is listed once with its test status, then checked in turn with the repair made.
Owners and landlords of older homes, and parents in a house where painted windows, doors or stairs may contain lead.
2 pages, in this order:
Printed under the title: Painted surfaces known or presumed to contain lead listed once, windows, doors and stairs first, then each condition check with what was found, what was done and who did it.
This records condition, not lead. Treat a surface as presumed lead until a test says otherwise. Do not dry-sand, burn off or scrape suspect paint to see what is underneath, and keep children and pets away from chips and dust. Whether repairs need a certified contractor, and what is owed to tenants, depends on the rules where you are: check them first.

From a test: a laboratory analysis of a paint chip, an inspection by a qualified inspector, or a test kit recognised where you live. Until then, older paint is best treated as presumed lead, which is why the sheet has a status column rather than assuming either way.
The sheet leaves that to you and to any rules for rented homes where you are. Some health departments advise rental owners to check every year and whenever a new occupant moves in. Whatever you choose, write each check as a dated line.
Rules on who may disturb lead paint, and how, differ between countries and can be strict for rented homes. Check them before starting, and never dry-sand, burn off or power-wash paint that may contain lead.

Record what went on which wall so a touch-up in three years is one trip to one shop instead of an afternoon of matching.
Record a property visit properly, with space for photograph references and both signatures, so the visit is evidenced.
Record a mid-tenancy inspection properly, separating what you need to repair from what the tenant needs to address, with the notice you gave written at the top.
