What this sheet is for
Show for every animal you moved where it came from, how it travelled, what paperwork you saw and who took it at the other end.
Who it is for
Pet taxi drivers, animal couriers, rescue transport volunteers and vets or kennels running their own collections.
How to use it
- Fill in the date, driver, vehicle and phone before leaving.
- Work through the pre-departure checks and tick each one.
- Log each animal as it is loaded: time, from, to, how it was restrained and what paperwork was seen.
- Have the person receiving the animal sign the row.
- Note anything that went wrong and whether the vehicle was cleaned after the run.

Questions
Why does the sheet not say what documents an animal needs to travel with?
Because that depends on the species, the journey and the rules where you are, and it changes. The papers column is for recording what you checked; what you have to check is yours to find out.
What about temperature in the vehicle?
The checklist asks whether ventilation or climate control was working. What is safe for a given animal on a given day is your judgement and your insurer's conditions, not a figure this sheet can print.
