Account for every dog, every key and every incident on the round, and hand a regular dog to a different walker without anything being lost.
Dog walkers, dog day-care collection rounds and anyone walking several owners' dogs in a day.
One page, top to bottom:
Printed under the title: One walker, one day — every dog collected, where it walked, what it did, when it went back and whether the key did too.
The owner's walk, on paper. Lead or off-lead, alone or in a group, how long, feed on return, which dogs it cannot be with — each owner's instructions are theirs, not the sheet's. Write the round so a different walker could do it tomorrow.
Write-on lines: Date · Walker · Vehicle & reg. · Phone on the round
Table, 9 rows: Out · Dog & address · Access · Walk · Lead · Toilet · Fed · Back · Notes
Write-on lines: All keys returned / secured · Incidents or injuries reported to · Dogs that could not be walked & why · Mileage start · Mileage end · Fuel / expenses · Signed · Checked by
Because each owner decides how long and with whom their dog walks, and the rules for walking several dogs at once differ from place to place. The walk column records what was done for that dog; the owner's instructions are the reference.
Per day. This is the round, in order, with every dog on it. A dog's own history over time belongs on its owner's records.
Give a sitter everything they need so they do not have to ring you, including the part most people leave out — what this animal is like when nothing is wrong.
Show for every animal you moved where it came from, how it travelled, what paperwork you saw and who took it at the other end.
Keep a record of what you actually worked on with a dog, and of what the dog has already met. The notes column is the point — a reaction written down at the time is worth more later than a tick.
Put everything a neighbour, a sitter or an emergency responder would need onto one page and leave it where a stranger would look. It answers the questions that get asked when you are not there to be asked.

Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee.
