Who this is for
One page for a pet's whole medical history. The top carries the facts a vet, kennel or insurer asks for, the middle records each vaccination with the date it was given and the date the vet said it falls due, and the lower table keeps treatments, operations and ongoing conditions in date order.
Owners of a dog, cat, rabbit or any other pet with a vaccination card that keeps going missing, fosterers handing an animal on, and anyone booking kennels, cattery or travel where proof of vaccination is asked for.
What this helps you track
- Which vaccinations the pet has had, when, and when the vet said the next is due
- Batch numbers and the practice that gave each one, for a kennel or travel check
- Operations, treatments and diagnoses in one dated list
- Microchip, insurance and vet details in one place for a new sitter or a new vet
How to use it
- Fill in the pet's details at the top, including the microchip number from its registration paperwork.
- Copy each vaccination from the vet's card or invoice: what it was, the date given, and the next due date the vet wrote down.
- Add the batch number or sticker reference if the practice gives one — some kennels and travel schemes ask for it.
- Record each treatment, operation or diagnosis in the lower table as it happens, with the vet who saw the animal.
- Keep the sheet with the pet's paperwork, or fill it in online and save it in your own browser.

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.
How to print
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
Questions
Does this tell me when the next vaccination is due?
No. The next due column is for the date your vet gives you. Vaccine schedules differ by vaccine, country, age and the animal's history, so the sheet carries your vet's date and not one of ours.
Is this an official vaccination certificate?
No. It is a record for your own use. Kennels, catteries, travel schemes and shows will usually want the vet-signed card or certificate; this sheet helps you keep track of what is on it and when it needs updating.
What goes in the treatments and conditions table?
Anything that a future vet would want to know: operations, injuries, diagnoses, courses of treatment, parasite treatments, reactions to a medicine. Day-to-day tablets are better kept on the medication log.
Can I use one sheet for several pets?
One sheet per animal is safer, because vaccinations and conditions are individual. Print or save one copy for each.

Cat Health Record
Hold a cat's identification, insurance and treatment history on one page that a cattery, a sitter or a new practice can read. It has room for the things that are specific to cats and get left off general pet forms.
Multi-Pet Household Record
Get every animal in the house onto one page, so a sitter, a new vet or an insurer can be handed a single sheet instead of a folder. It also carries the feeding and medication detail that stops the wrong bowl going to the wrong animal.
