Who this is for

A dog's vaccination history on one sheet. Each line takes what was given, the date, the practice that gave it, the batch reference if one is supplied, and the next due date written down at the appointment — so the card in the drawer stops being the only copy, and the second table records every time somebody asked to see it.

Dog owners who want their own copy of what is on the vaccination card, people whose dog goes to kennels, daycare, training classes or a groomer that asks for proof, and anyone taking on a rescue dog with a partial history.

What this helps you track

  • What your dog has been given and on what date, in one list
  • The next due date your vet wrote down, in a column you can scan
  • Batch or sticker references, which some kennels and travel schemes ask for
  • Which practice gave each one, useful after a change of vet
  • Every time proof was asked for and what you showed

How to use it

  1. Fill in the dog's details at the top — name, breed, date of birth, colour and markings, and the microchip number from the registration paperwork.
  2. Copy each entry from the vaccination card or the practice invoice: what was given, the date, the vet or practice and the batch reference if there is one.
  3. Write the next due date exactly as your vet gave it. Leave the column blank rather than guessing.
  4. When a kennel, class or groomer asks for proof, add a line to the lower table so you know what has been seen and when.
  5. Keep the sheet with the card, or fill it in online and save it in your own browser.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Does the sheet work out when the next booster is due?

No, and it never guesses. The next due column is for the date your vet gives you at the appointment. What a dog is given, and how often, depends on the vaccine, the country, the dog's age and its history.

Can I use this instead of the vaccination card?

No. Kennels, catteries, classes and travel schemes normally want the vet-signed card or certificate itself. This is your own copy of what is on it, so you can see at a glance what is coming up without hunting for the card.

My rescue dog's history is incomplete. What do I write?

Write what you have and mark the gaps as unknown. Take the sheet to your vet and ask what they want to do about the missing entries — an honest gap is more useful than a filled-in guess.

Why record the batch number?

Because some boarding kennels, insurers and travel schemes ask for it, and because it matters if a batch is ever queried. If your practice does not give you one, leave the column empty.

Is one sheet enough for two dogs?

Keep one per dog. Vaccination histories are individual and mixing two of them on a page is how the wrong date ends up against the wrong animal.
