The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
No. There are no tables, limits, ascent rates or surface intervals printed anywhere on this sheet, deliberately. Dives are planned with your computer or tables, your training and your buddy. This records the dive after it happened.
That is for them to say. Agencies and clubs set their own requirements for logged and verified dives, and some require their own book. If theirs conflicts with this, theirs is the one that counts.

Record what you actually did in each session, so progress over a block is something you can look at rather than something you try to remember.
