The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Keep an orderly account of each day on the ground, so what was taken and who was out can be shown to the permission holder without relying on memory.
Rough shooters, pest controllers and stalkers working on someone else's permission, and anyone running a day for guests.
One page, top to bottom:
Printed under the title: A day's record — the ground, whose permission it is, who was out, the briefing given, and what was shot, by whom and where.
Check the rules where you are. What may be shot, when, and what paperwork has to be held differs everywhere — check the rules where you are, and keep to the conditions the permission itself sets. No species, seasons or numbers are printed on this sheet; you write in what applied on your ground on the day.
Write-on lines: Date · Ground or estate · Permission holder · Weather and conditions
Table, 4 rows: Name · Certificate seen · Insurance seen · Firearm or gun used · Briefed · Phone
Table, 7 rows: Time · Drive, field or area · Quarry · Number · By whom · Retrieved · Where it went · Notes
Because what may be shot and when differs by country, by region and often by the terms of the permission itself. Check the rules where you are and write in what applied on your ground; a printed list would be wrong somewhere.
Because the briefing is the thing everybody remembers differently afterwards. A tick against each name, dated, says it was given before anyone loaded.

Record a risk assessment you have carried out yourself, in a layout that captures the part people forget: who is doing the further action, by when, and when it gets reviewed.
Capture an incident properly on the day, in the affected person's own words, with the witness details and equipment notes that become impossible to gather later.
Turn the fire into somebody's named responsibility, with the before-and-after checks recorded — particularly where a group or other people's children are involved.
