Take an auto belay with worn webbing or an expired label off the wall the day it is found. Every unit's checks are on one dated record.
Climbing walls, leisure centres, schools and activity centres with auto belays.
2 pages, in this order:
Printed under the title: Each auto belay on the wall listed with its serial and recertification date, then the periodic checks unit by unit, and a daily line for the visual check. The maker's manual sets how; this sheet records it.
Write-on lines: Centre or wall · Responsible person · Manual version used
Table, 6 rows: Unit · Position on the wall · Make & model · Serial · In service · Recertified · Due · Karabiner
Also printed: Body and labels · Karabiner · Webbing
Table, 7 rows: Date · Unit · Casing · Labels · Cert · Gate · Swivel · Webbing · Retracts · Result · Action · By
Also printed: Unit, numbered as on page one
Table, 20 rows: Date · 1 · 2 · 3 · 4 · 5 · 6 · Taken out of use · By
Ruled lines: Notes: anything reported by climbers or staff
The sheet does not say, because the maker's manual sets the daily, periodic and recertification checks for each model. Follow the manual for your units, and use this sheet to show they were done.
Use a second copy for units 7 to 12, and write the unit numbers on it. Keeping six to a sheet leaves the columns wide enough to write in.

Retire a rope on its history, not on how it looks. Every day out, every fall held and every inspection is written against the one rope.
Build a record of what you have actually climbed and how, so progress shows up as routes on paper rather than as a feeling about how the year went.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
