A free printable fruit pest trap record: each trap with its lure and position, lure and liner changes, every count in a dated grid, and your biofix date.

Read the start of a pest flight off your own counts instead of guessing it. Empty checks are written down too, because a run of zeros is what dates the first catch.
Fruit growers, community orchards and allotment holders using pheromone or bait traps.
2 pages, in this order:
Printed under the title: Each trap registered once, then every count in a dated grid with lure and liner changes logged, so the date catches began in earnest is read off the record instead of guessed.
Count, then follow your own source. What counts as the first sustained catch (the biofix), what you do from that date and how often lures and liners are changed come from your advisory service or the trap maker; none of those is printed here. Write down every check, including the empty ones: a run of zeros is what shows when the flight began.
Write-on lines: Orchard or garden · Season · Checked by · Advisory source followed
Also printed: Trap · Pest targeted · Trap type · Lure or bait · Hung in · Set on · Taken in · delta, bucket, cup · and its lot or date · tree, row or block · date
Also printed: Pest · Biofix · Traps · How you defined it · Source · What follows · date · nos. · first sustained catch · what you do from it
Also printed: Count in each trap · Date · Days · Changed · Notes · checked · since last · initials · trap: L, C, B · weather, damage · Total · Changed: write the trap number with L for a new lure, C for a new liner or card, B for fresh bait. Catches can jump or drop after a change.
The date a grower takes as the start of a pest's flight, usually the first sustained catch in the traps; timings for scouting or treatment are then counted from it. What counts as sustained differs between advisory services, so the sheet asks you to write your source's definition.
Thresholds differ by pest, crop, region and trap, and they change as advice is updated. Take any figure from your advisory service and note it beside the biofix, so your decision rests on a current source.

Keep a history for every tree, so the one that never crops and the one that carries the whole harvest are both on paper rather than in your head.
Record applications properly at the time, including the conditions, so the record stands up when a buyer, an inspector or a neighbour asks about it.
Judge frost risk and losses on your own trees, at the stage they have actually reached. Each cold night and the damage check after it sit on the same page.
