Trace any batch back to its supplier or forward to its buyer if a pest or disease turns up. Keeping the passport details and the arrival check together makes the record quick to search.
Nurseries, garden centres, growers and plant sellers who receive and pass on plants with plant passports.
2 pages, in this order:
Printed under the title: Plants bought in with their passport details, the check made on arrival and any time held apart; then on page two the plants passed on, so a batch can be traced either way if a pest or disease turns up.
Check the rules where you trade. Who must issue or keep plant passports, what goes on them and how long the records are kept are set by the plant health rules where you trade, and they differ between countries. Check the current guidance from your plant health authority. This sheet keeps the details; it is not a passport and does not replace one.

No. A plant passport is an official label issued by an authorised operator. This sheet keeps the details from passports you receive and issue, so a batch can be traced; it does not replace the passport or any record the rules ask you to keep.

Many growers keep incoming plants away from their own stock for a while so that a pest or disease can show itself before it spreads. How long, and whether you do it, is your own decision; the sheet records it.
Record what struck and what did not, so your own conditions become the reference instead of a general table.
Build a picture of what turns up on your plot and when, and see whether what you tried last time actually made any difference.
Tie every label in the ground to its record with one number that is never reused. The register keeps each plant's name, source, position and status, and page two logs every move and relabelling.
