Keep the certificates and service dates for everything sent away for testing. They are then in one file for the surveyor, the insurer or a charter check.
Owners and skippers of boats that carry a liferaft, inflatable lifejackets, beacons or serviced extinguishers — cruising yachts, motor boats, charter and club boats.
2 pages, in this order:
Printed under the title: The items on a boat that are sent away, tested or recertified — liferaft, hydrostatic release, lifejacket cylinders, beacons, extinguishers — each with its own service history, in one place for when the surveyor or the insurer asks.
Service intervals are the maker's and the station's, not ours. How often each item has to be serviced, who may do it and whether it must be done at all depends on the maker, the type of boat and the rules where you sail — the interval is a blank here, for the figure from your service station or manufacturer. The Boat Safety Equipment Check is the inventory of what is aboard; this is the servicing file for the items that leave the boat.
Write-on lines: Vessel · Service station used · Phone / email
Write-on lines: Make & model · Serial no. · Persons · Pack type · Date of manufacture · Stowed where · Hydrostatic release — make & fitted · HRU replace by · Painter secured to
Table, 5 rows: Date sent · Station · Work & findings · Certificate no. · Next due · Cost · Back aboard
Ruled lines: Notes — what the station found, what was condemned, what to budget for
Table, 21 rows: Date · Item & serial · Serviced / tested by · Result · Certificate · Next due · Cost
The maker sets a service interval, the service station may vary it depending on the pack and its age, and some waters and types of use have their own rules. The Next due column takes the date from your own certificate.
That sheet is the inventory — what is aboard and when each item expires, checked at the dock. This is the servicing file for the items that go away to be tested: who did it, what they found and the certificate they issued.

Leave a shore contact with everything they need if the boat is overdue. It also makes you write the passage plan down before you cast off.
Keep a dated record of monthly visual checks, and separate them clearly from the professional servicing that premises usually also need.
Put the autumn lay-up jobs and the spring launch checks on the same page, so what you did in October is in front of you in April.
