Free printable puppy toilet training log — time, what happened (wee, poo, accident, meal, sleep, play), where, and notes. Two pages, PDF and browser versions.

A running diary of a puppy's day for house-training. Every wee, poo, accident, meal, drink, sleep and play session goes on a line with the time and the place, and within a few days the pattern that tells you when to go outside is on the page.
New puppy owners in the first weeks at home, families sharing the job between several people, and anyone whose vet or trainer has asked for a record of accidents.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
Often, and more often than feels necessary at first — but the exact timing depends on the puppy's age and the pattern in this log. Your vet or trainer can say more; the sheet makes no rule of its own.
Because toileting follows them. Without the meal and nap times on the same page, the accidents look random. With them, they usually do not.
Anything indoors that was not on the pad or in the place you intended. Record it without fuss — the log is for spotting patterns, not for blame.
A busy puppy can fill a page in a day. The second page is the same table again; print as many as you need.
Which skills are reliable, which are getting there and which have not started.
What the pet eats at each feed, and how much, in your own words or your vet's.
Which vaccinations the pet has had, when, and when the vet said the next is due.
Printable sheets, workbooks and tools on the same subject.
Keep the first year of a puppy's treatments in one place, with the dates your vet gave you written down before you forget them. The weight at each treatment is worth recording too, because a puppy's changes week to week and nobody remembers it afterwards.
Keep a record of what you actually worked on with a dog, and of what the dog has already met. The notes column is the point — a reaction written down at the time is worth more later than a tick.

Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
