What this sheet is for

Keep a document's history with the document, so the copy in somebody's drawer says which version it is and who approved it.

Who it is for

Small businesses writing procedures, committees maintaining policies, contractors issuing method statements, and anyone whose documents get printed and circulated.

How to use it

  1. Staple it to the front of the document, not into a separate file.
  2. Say what changed, not just that something did.
  3. Record who approved the version and when.
  4. Keep the distribution list current, and mark copies withdrawn when you replace them.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Does this meet a document control standard?

It sets no standard and claims none. If you are working to a specific one, use the fields that standard requires — this is a plain pro-forma.
