One page, top to bottom:
Printed under the title: When a fridge or freezer fails: what happened to the unit, then every item that was in it, the state it was found in and what was decided for it, so each call is on record.
Your procedure decides. No safe temperatures or times are printed. Decide each item by your own food safety procedure and the pack, as some foods must not be refrozen once thawed. Move what is still frozen or chilled first, and write each decision as it is made. If you will claim on insurance, note values and keep photos.

Because that depends on the food, how long it was out of temperature, what the pack says about refreezing, and your own food safety procedure. No single temperature or time on a sheet could cover every item. The sheet records what was found and the decision made, item by item.
No, but it gives you what an insurer usually asks for: what was lost, how much, its value and why it was thrown away. Keep photos and the engineer's report with it.

Keep every fridge and freezer clean and checked to its own manual. A dated record of coil cleans and seal checks also backs up a warranty claim if a unit fails.
Record what gets thrown away and why, which is usually the fastest way to find where a kitchen's margin is going.
Give every fault on a fridge, fryer, oven or glass washer one line, so nothing is reported twice or forgotten once, and the manager can see what is still open.
