What this sheet is for

Record a change to a job at the moment it is agreed, priced and signed, instead of arguing about it at the end when nobody's account matches.

Who it is for

Builders, electricians, plumbers, joiners, groundworkers, small contractors and anyone whose customer keeps asking for one more thing.

How to use it

  1. Raise one sheet per change. A sheet covering four changes gets disputed four times.
  2. Record who requested it and how — in person, by phone, by email.
  3. Price it before the work is built where you can, and say plainly when you cannot.
  4. Get it signed before starting, or record in writing that you were told to proceed without a signature.
  5. Keep the signed copy with the contract, not in the van.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Does this entitle me to be paid for the extra work?

No sheet can do that. What you are entitled to comes from your contract, and the sheet prints no terms, percentages or entitlements. What it does is make sure the change, the price and the agreement exist in writing — which is usually the thing missing when these arguments happen. If there is real money in it, take advice.

The customer will not sign. What now?

Write down that you asked, when, and what they said, then confirm it by email or message the same day so there is a dated record outside your own paperwork. That is worth considerably more than nothing, which is the usual alternative.
