Radon Test Record
Keep every radon test on one home together, so a follow-up is compared like with like. Each line also says why the test was done and what was decided after it.

Catch a stopped radon fan or an iced-up vent exit when it is checked, not months later. Every check compares the manometer with the reading on its label.
Homeowners and landlords with an active radon fan system, and anyone buying a home that has one.
2 pages, in this order:
Printed under the title: The owner's regular look at a radon fan system: the manometer against the reading on its label, the alarm, the tubing, the fan and the vent exit, then the retests and service visits on the back.
The label is your reference. No readings or limits are printed. Copy the manometer reading written on the system's label at installation into the box below and compare every check with it: a reading that has moved, or fluid sitting level on both sides, is worth a call to the installer. Retest radon after installation and from time to time, as advised locally.
Write-on lines: Installer · Phone · Installed on · Warranty ends · Fan make and model · Fan location · Suction points · Vent exits at · Reading on the label · Monitor or alarm type · Fan circuit or breaker

Whatever the installer wrote on its label when the system was set up: each system is different, so the sheet prints no figure. What matters is change. A reading that has moved, or fluid sitting level on both sides, means the fan may not be drawing, and the installer should look at it.
Ask your installer and follow your health authority's advice; the sheet leaves the interval to them. The check table has room for a regular look at the manometer, and the retest table for the tests after installation and later on.

Prove the pump still works before the water arrives, including the backup that nobody ever tests.
Remember who did what, what it cost, and who was worth calling back — and have a dated record if a dispute ever starts.
Turn a failure into a claim instead of a bill, by knowing what is still covered before you call someone out.
