Who this is for

A joining form for a club or community group. It collects the new member's details, membership type, emergency contact and consents, with an office box for subs.

Membership secretaries and committees of clubs, societies and community groups who want one tidy form for everyone who joins.

What this helps you track

  • The new member's contact details and membership type
  • An emergency contact for club activities
  • Which sections or activities they want to take part in
  • The consents your group asks for, in your own wording
  • When subs were paid, how, and the membership number issued

How to use it

  1. Write in the membership types your club offers before printing, or leave the field for the applicant.
  2. Replace the consent lines with your group's own wording, including how you hold members' details.
  3. Hand the form to the new member to complete, sign and date.
  4. When it comes back with the subs, fill in the office-use section and give a membership number.
  5. Add the member to your register and tick when the welcome has been sent.
  6. Keep completed forms securely and only for as long as your group's own policy says.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Do we need to ask for date of birth?

Only if your club needs it, for example for junior or senior categories. It is marked optional; leave it out if you have no reason to collect it.

Is the consent wording on the form enough for data protection?

The lines are placeholders to be replaced with your group's own wording. What you must tell members and record depends on the rules where you are, so check them and write your own statements.

How much should membership cost?

The form supplies no fees. Write your own membership types and subs on the form or on a separate sheet, and record the amount actually paid in the office section.

Can a family join on one form?

Use the notes area to list other household members joining, or give each person their own form if your register needs separate emergency contacts.
