A place to keep what you have been told about taking an animal abroad. The top holds the document references and who issued them, the requirements table records each thing your vet or the authority said this journey needs and the date it was done, and the journeys table is the history of where the animal has actually been.
Owners travelling with a pet, people moving abroad with an animal, and anyone keeping the documents for a dog, cat or ferret that crosses a border more than once.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
No, and it deliberately does not try. Entry requirements differ by country, by species, by where you are travelling from, and they change. This sheet records what your vet and the destination's own authority told you.
From the official source for the country you are entering and from your own vet, close to the date of travel. Requirements are often time-sensitive, which is why the table has both a date told and a date done.
No. Schemes, documents and their names vary, and a document accepted on one route may not be on another. The document columns take whatever you actually hold.
Because a border question is easier to answer with dates, routes and references than from memory, and because a returning journey often depends on what was done on the outward one.
Yes. Give transit countries their own lines in the requirements table — what a route needs is not always what the destination needs.

The chip number written somewhere other than a database you cannot log into.
What was given, on what date, and by whom.
Every leg of the trip with its date, time, booking reference and whether it is confirmed.
Printable sheets, workbooks and tools on the same subject.
Record journeys and the pre-loading checks, which is both good practice and, for longer or commercial journeys, often a requirement.
Put everything a neighbour, a sitter or an emergency responder would need onto one page and leave it where a stranger would look. It answers the questions that get asked when you are not there to be asked.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
