Who this is for

A running history of every parasite treatment an animal has had. Each line takes the product exactly as it was supplied, the date it was given, the animal's weight that day, who gave it and the next due date your vet told you — so the question of whether the dog is up to date has a written answer instead of a guess.

Owners of a dog, cat or other pet on any kind of worm, flea or tick treatment, households where more than one person gives it, and fosterers or new owners taking on an animal with a part-known history.

What this helps you track

  • What was given, on what date, and by whom
  • The weight the animal was when it was given, which is what the product was supplied against
  • The next due date exactly as your vet gave it
  • Where the treatment came from — practice, prescription or shop
  • Anything you noticed afterwards, ready for the next appointment

How to use it

  1. Fill in the animal's details at the top, with the vet practice and the microchip number.
  2. When a treatment is given, copy the product name and strength exactly as it is written on the packet or the label.
  3. Weigh the animal if the product was supplied against a weight, and write that weight on the line.
  4. Write the next due date your vet or the supplying pharmacy gave you. If you were not given one, leave it blank and ask.
  5. Initial the line, and note anything you saw afterwards in the last column.
  6. Keep the sheet with the animal's paperwork, or fill it in online and save it in your own browser. The second page is the same table again.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

How often should I worm or treat my pet?

We do not say, and no sheet should. It depends on the animal, its age, where you live, whether it hunts, and the product itself. Your vet sets it and the next due column carries their date.

Which product should I use?

That is a question for your vet or a qualified prescriber. The product column is a blank for copying in what you were actually supplied; no product is named anywhere on the sheet.

Why record the weight?

Because many treatments are supplied against a weight band, so a record of the weight on the day explains why that pack was used. Use your own scales and your own unit, consistently.

What if a dose was spat out, rubbed off or refused?

Write it on the line as it happened and ring the practice rather than repeating it. An honest record is more use to a vet than a tidy one.

Can I record more than one animal on a sheet?

Keep one per animal. Weights and due dates are individual, and one shared sheet is how a treatment ends up logged against the wrong pet.
