Free pet weight chart — each weighing with the change since the last one and the body condition score your vet gave. PDF, Excel and browser versions.
2 pages. The A4 and US Letter files are laid out identically, so neither is shrunk to fit.

The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.
A weight record that shows the direction of travel. Every weighing goes in with the date, the figure and where it was taken, and the change column subtracts the previous reading so a slow drift is visible before anyone notices it by eye.
Owners of a pet on a weight plan agreed with the vet, anyone watching weight through an illness or old age, and multi-person households where the animal is weighed by whoever is at the practice that week.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
That is a question for your vet, and the sheet deliberately carries no target weight, no range and no condition scale of its own. There is a field for the target your vet sets, if they set one.
A hands-on assessment a vet or nurse makes, using a scale their practice works to. Write down the score and the scale they used; the sheet supplies neither, because several different scales are in use.
Whatever your practice suggests. Weighing far more often than the animal can change mostly records the scales rather than the pet.
There is no previous reading to subtract from. The change column starts working at the second weighing.
Yes, in whatever unit suits — grams for a small animal, kilograms or pounds for a larger one. Keep one sheet per animal and one unit per sheet.
Check-ups and tests with the result you were given and the next date.
Every appointment in date order with the reason you went.
Every vet visit in the first year, with what was done and what comes next.
What the pet eats at each feed, and how much, in your own words or your vet's.
Printable sheets, workbooks and tools on the same subject.
Track weight against what the animal is actually being fed, so a change in one can be read against the other. A single weighing tells you very little; a column of them taken the same way each time is something a vet can use.
Give any small pet a weekly weight record, which is the earliest warning available for animals that hide illness by instinct.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
Opens PayPal in a new tab. Any amount, and nothing changes if you would rather not.
