The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.
The sheet a kennel or cattery keeps for one animal for one stay. It holds the owner's contact numbers and vet consent, the feeding and medication instructions exactly as the owner gave them, a line a day for what was fed, given and noticed, and the charges for the stay adding themselves up.
Boarding kennels, catteries, home boarders and small daycare businesses, and owners filling in the instruction half before they drop the animal off.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
No. Everything in the instruction table comes from the owner, and any medication is given as supplied and labelled. No amounts, frequencies or doses appear anywhere on the sheet.
It is a record of what the owner told you, not a legal consent form. What a boarding business needs by way of consent, insurance and licensing differs by country and sometimes by council — check the rules where you are.
That is why there is a second contact and a line for what the owner agreed should happen in an emergency. Fill both in at booking, not on the day.
Use one per animal even when they share a pen, so medication and daily notes cannot be attached to the wrong dog or cat. Note on each that they board together.
Only if your rate does. The sheet multiplies whatever you enter and totals it; how you handle tax is yours.
Arrival and departure times, and who handed the dog over at each end.
The daily routine, in order and with times, so nothing is forgotten by day three.
Who the animal belongs to while it is with you, and who to ring about it.
What your dog has been given and on what date, in one list.

Give a sitter everything they need so they do not have to ring you, including the part most people leave out — what this animal is like when nothing is wrong.
Put everything a neighbour, a sitter or an emergency responder would need onto one page and leave it where a stranger would look. It answers the questions that get asked when you are not there to be asked.

Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
