The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Who this is for

The general register for a group of animals in one person's care. The register itself is a running line per treatment — which animal, what was given as supplied, the date, who gave it and the next due date you were told, with the days to that date worked out; the table at the foot lists the animals and how each one is identified.

Multi-pet households, small rescues, catteries, kennels, sanctuaries, yards and smallholdings that keep several animals and need one book rather than a folder of separate cards.

What this helps you track

  • Every animal in your care identified the same way, by name, chip or tag
  • Which animal was given what, on what date and by whom
  • The batch or reference where the supplier gives one
  • The next due date as the vet or prescriber gave it, per treatment
  • How many days are left on each, and which lines have no date at all

How to use it

  1. Fill in the premises or household, who keeps the register, the period it covers and the vet practice at the top.
  2. List the animals in the table at the foot with the identification you actually use — name, microchip, tag, ring or pen number.
  3. Add a line to the register every time anything is given: the animal, what it was as supplied, the date and who gave it.
  4. Copy the batch or reference from the packaging when there is one, and write the next due date exactly as your vet or prescriber gave it.
  5. The days column, the totals and the summary figures work themselves out on screen and in the workbook.
  6. Keep the register with the medicines, and start a new sheet each period rather than squeezing a second year on.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser
  • Excel / Google Sheets workbook

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Does the register tell me what to give or how often?

No. It names no product, sets no dose and works out no interval. Everything on a line is what was actually supplied and what your vet or prescriber told you.

Is this the medicines record my inspection or assurance scheme wants?

Possibly not. What must be recorded, for how long and in what form differs by country, by species and by scheme — check the rules where you are and keep whatever they specify. This is a working register, not a compliance document.

What is the animals table for?

So that the identification is written once and used consistently. Registers go wrong when the same animal is called three different things on three different lines.

Can it mix species?

Yes — that is the point of it. A household with dogs, cats and rabbits, or a yard with horses and a farm cat, can keep one register with a species column.

What does the days column do when I leave the due date blank?

Nothing — it stays empty. A blank due date is a legitimate answer when none was given, and the register never invents one.
