Free grooming salon client record — dog details, coat and handling notes, services given with prices and visit history that totals. PDF, Excel, browser.
2 pages. The A4 and US Letter files are laid out identically, so neither is shrunk to fit.

Fillable PDF type and save
Spreadsheet formulas included
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.
One card per dog, kept by the salon. The top holds the animal, the owner and the people to ring; the two note boxes hold the coat and the handling — the things a groomer learns in the first half hour and should never have to learn twice — and the visit table records what was done, what it came to and when the owner was told to come back.
Grooming salons, mobile groomers and home groomers keeping a card per client, and owners who want their own copy of what was done to the coat and what it cost.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
That is a judgement for the groomer and the owner, based on the coat, the breed and how the dog lives. The rebooking column takes the date you gave, not an interval of ours.
Because a matted coat changes what can be done and what it costs, and because a written note made before work starts settles most disagreements afterwards. Take a photograph too if your salon does that.
Yes, with the date. Groomers see skin, ears, claws and teeth more closely than most owners do. Tell the owner to speak to their vet; do not offer a diagnosis or a treatment.
It multiplies and totals whatever you enter. Whether your prices include tax, and at what rate, is set by your business and the rules where you are.
Keep one per dog. Coats, temperaments and prices differ, and a shared card is how the wrong clip ends up on the wrong dog.
Arrival and departure times, and who handed the dog over at each end.
What was given, on what date, and by whom.
When the pet was last groomed and what was done — full groom, bath, clip, nails, ears.
Every customer's phone, email and address on one sheet.
Printable sheets, workbooks and tools on the same subject.
Keep one card per customer so the history of what was done, what it cost and when the next visit is due lives in one place.
Put everything a neighbour, a sitter or an emergency responder would need onto one page and leave it where a stranger would look. It answers the questions that get asked when you are not there to be asked.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
