What this sheet is for

Keep a claim's own timeline and costs written down as they happen, so a stalled or disputed claim has a dated record behind it rather than a memory of what was promised.

Who it is for

Homeowners and landlords part-way through an insurance claim for a leak, fire, storm or theft.

How to use it

  1. Fill in the incident and policy details as soon as the claim opens.
  2. Log every call, email or visit the day it happens, not from memory afterwards.
  3. Record what was actually said or agreed, not what you hoped was agreed.
  4. Keep this with the receipts, quotes and photographs each row refers to.

What is on the sheet

2 pages, in this order:

Page 1

Printed under the title: A dated record of an insurance claim in progress — what happened, who you spoke to, what was agreed and what it cost — kept from the day it happened rather than reconstructed when the claim stalls.

This does not tell you what is covered. This sheet does not say what your policy covers or how a claim is assessed — those are set by your policy wording and your insurer, and they differ. It exists so the dates, names and figures are all in one place when you are asked for them, rather than relying on memory.

Write-on lines: Property · Incident date · Incident type · Insurer · Policy number · Claim reference

Contact log — every call, email or visit, in order

Table, 6 rows: Date · Who — insurer, adjuster, contractor · Method · What was said or agreed · By whom

Damage and cost — one row per item or area of damage

Table, 21 rows: Date · Item / area · Description of damage · Photo / file ref. · Quoted / paid · Contractor · Receipt kept

Questions

Do I still need to keep the actual receipts and photographs?

Yes — this sheet is an index of what exists and where, not a replacement for the receipt, invoice or photograph itself. Keep those together, and note here that you have them.

What if my insurer asks for something not on here?

Insurers set their own requirements and they differ by insurer and by claim type. If they ask for something specific, add it as its own row — this sheet is a structure, not a limit.
