What this sheet is for

Do the same groom the owner liked last time without asking them to describe it, and have a written note of what you saw and told them about skin, ears or nails.

Who it is for

Dog groomers, mobile groomers and salons with more than a handful of regular dogs.

How to use it

  1. Open a card for each dog at the first visit, with the owner's contact details and their vet.
  2. Write in what the owner tells you about health, allergies and medication, in their words.
  3. Note how the dog is with the dryer, nails and face, and whether a muzzle or a signed consent is needed.
  4. Log each visit: the service, the blade or length agreed, the coat as found and anything you noticed and told the owner.
  5. Book the next visit in the last column so it is on the card.

What is on the sheet

One page, top to bottom:

Printed under the title: A client card for one dog — owner, coat, disclosed health and temperament, then a line per visit for the groom done.

Noticed and told, not diagnosed. The clip and length are whatever the owner agreed on the day, and anything about skin, ears, lumps or teeth is something you noticed and told them — not a diagnosis. Write it down as seen, and the owner's vet decides what it means.

Write-on lines: Dog's name · Breed · Age · Sex / neutered · Weight (owner's figure) · Owner · Phone · Email · Client since · Coat type · Usual style / length (owner's choice) · Owner's vet & phone · Health disclosed by owner — skin, joints, heart, allergies, medication · Temperament — dryer, nails, feet, face, muzzle needed? · Consent for handling / muzzle signed

Visits — one line per appointment

Questions

Why does the sheet not suggest blade sizes or how often to groom?

Because the clip is whatever the owner and the groomer agreed for that dog and that coat, and how often depends on the breed, the coat and the household. The columns hold your figures, not ours.

Is the 'noticed' column a health record?

No. It is a note that you saw something — a sore patch, a lump, an ear that smelt — and told the owner. Deciding what it is belongs to their vet.

Track weight against what the animal is actually being fed, so a change in one can be read against the other. A single weighing tells you very little; a column of them taken the same way each time is something a vet can use.
