Carry what a guest told you at the desk to the kitchen and to housekeeping without it being retold from memory, and show it was confirmed with them.
Reception and front-of-house staff in hotels and guest houses, and owners who do the check-in themselves.
One page, top to bottom:
Printed under the title: A card for one room's stay — every allergy, intolerance and special request the guests told you about, who it was passed to and whether they confirmed they had it.
Read it back to the guest. Record allergies in the guest's own words and read them back before the guest leaves the desk. This card carries the information to the kitchen and to housekeeping; what a dish contains is checked against your own allergen record.
Write-on lines: Room · Lead guest name · Arrival · Departure · Guests in room · Taken by
Table, 5 rows: Guest name · Allergy or intolerance · As described by the guest · Kitchen told · By · Guest confirmed
Table, 4 rows: Request · Passed to · Actioned by · Date · Guest told
Because a tick list invites a guest to pick the nearest match. Writing it in their own words — what they react to, how badly, what they avoid — gives the kitchen what it actually needs to check against its own allergen record.
Whoever your house says. The point is that one card follows the room, with the kitchen and housekeeping columns showing it has reached the people who act on it.
Get the kitchen a single list the night before, so the first pan goes on knowing who is eating, when, and what one guest cannot have.
Stop a request dying between the desk and the person who does it, and make sure the guest hears that it has been done.
Keep one grid that anybody taking an order can read, instead of a set of recipe cards nobody has time to look through during service.
Know at any point in the day who is in, who is still expected and which rooms are back with housekeeping, without opening a booking system.
