Who this is for

One meeting on one page, with the actions where they cannot be lost. The top holds the meeting itself — what it was, when, who came and why — the middle holds the notes by topic, and the action table at the foot gives each action an owner, a date and a tick.

Anyone who runs or sits in meetings and leaves them with a page of scribble and no clear list of who is doing what.

What this helps you track

  • What the meeting was for, who was there and when it happened
  • What was discussed and decided, topic by topic
  • Every action with a named owner and a due date
  • Which actions are done and which are still open at the next meeting

How to use it

  1. Fill in the title, the date, who attended and what the meeting was meant to achieve.
  2. As the meeting goes, write each topic on a row with what was said or decided beside it. Decisions are worth marking as such.
  3. The moment anyone agrees to do something, put it in the action table with their name and a date — before the conversation moves on.
  4. Read the actions back at the end so everyone hears their own name against their own task.
  5. Bring the sheet to the next meeting and tick the actions off first; the open ones start the agenda.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

How is this different from the minutes sheet?

Minutes are a formal record with agenda numbers and approvals. This is the working page for an ordinary meeting: notes by topic and an action table, nothing else.

What if there are more actions than rows?

Add rows in the browser version, or use the action log sheet on this site, which is built for a long-running list across many meetings.

Can I use it for a one-to-one?

You can, though the one-to-one meeting record on this site is laid out for that — this sheet suits a meeting with several people and several topics.

Should the notes be shared afterwards?

That is your call and your workplace's habit. The browser version downloads a completed PDF, which is the easiest thing to send.
