What this sheet is for
Keep a running record of first aid given so that supplies used and patterns of injury are both visible in one place.
Who it is for
Workplaces, clubs, halls, events, schools, sites and anyone keeping a first aid point.
How to use it
- Fill it in at the time, at the first aid point.
- Record supplies used — it is how the kit gets restocked before it is needed.
- Note whether a formal accident record was also completed.
- Keep the log secure; it holds health information about named people.
- Review it periodically for repeat causes.
What is on the sheet
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Questions
Does this sheet tell me how to give first aid?
No. It prints no first aid instruction of any kind and is not a substitute for training. It records treatment that has been given.
