Who this is for
A diary of what you watched. Each line records the date, the title, whether it was a film or a run of a series, where you saw it, who you watched with and what you thought. Over a year it becomes a record of your viewing you can look back on.
Film fans, box-set bingers and couples or families who like to remember what they have watched together and what was worth it.
What this helps you track
- Every film and series watched, with the date
- Which season and episodes of a series you got through
- Where you watched it: cinema, streaming, disc or broadcast
- Who you watched with
- Your own rating out of 5 and whether you would watch it again
How to use it
- Write your name, the year and the page number at the top.
- Add a line each time you finish a film or a sitting of a series, with the date and the title.
- Choose film or series, and for a series note the season and episodes watched.
- Record the release year, where you watched it and who with.
- Give it your own rating out of 5, tick rewatch if you would see it again, and add a few words.
- Keep the year's tally in the header boxes as each page fills, or use the browser version, which saves in your own browser.

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.
How to print
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
Questions
How do I log a series I watch over weeks?
Either a line per sitting with the episodes watched, or one line when you finish the season. The first gives a truer diary, the second keeps the page shorter; choose one way and keep to it so the tally counts like with like.
What do the tally boxes count?
Films and series finished so far this year. Update them when you finish a page, carrying the figure forward to the next page so the latest sheet always shows the year to date.
How should I rate things?
Out of 5, on your own scale. Some people use half marks; the column takes whatever you write. It is your record, so the rating means whatever you want it to.
Can I use it for a household?
Yes. The watched with column records who was there. For separate records, keep one log per person.
