Who the family are, and how to reach them. The sheet pairs a contact list with a simple three-generation grid, so an executor can work out who a will means by "my nieces" or "my grandchildren" and get in touch with each of them.
People putting their affairs in order, especially those with a large or scattered family, and executors trying to trace relatives named only by relationship.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
Wills often name people by relationship rather than by name, and executors are frequently not close family. A sheet like this saves weeks of tracing and reduces the chance of someone being missed.
It usually helps to, with a note saying so. An executor may still need to identify or contact them, and knowing the history in advance avoids an awkward surprise.
No. It is a map of the family and how to reach them. Who inherits depends on the will, if there is one, and on the rules where you are.
Three generations covers most wills: parents, siblings, children and grandchildren. If your will mentions cousins or great-grandchildren, add rows online or list them on the back.

Put the numbers somebody needs in one place, so a house-sitter, a babysitter, a visiting relative or a tenant is not searching for them at the worst moment.
Type your household’s emergency information once and print a clean sheet to pin up where it can be found.
