Who this is for

Every badge and certificate, in one list. Record each achievement from school, clubs and home with the level reached, whether there is a certificate or badge, and where it has been put, plus what your child is working towards next.

Parents who have a drawer of loose certificates, children who like to see how far they have come, and families who need a grade or award date for an application later.

What this helps you track

  • What your child achieved, when, and with which school or club
  • Grades, levels and stages reached in music, sport, swimming and more
  • Which achievements came with a certificate, badge or medal
  • Where each certificate is stored and whether it has been photographed
  • What your child is aiming for next

How to use it

  1. Write the child's name and the date you started the record.
  2. Add the goals your child is working towards in the table below the list, with who is helping.
  3. Each time an achievement comes home, add a line with the date, what it was, the organisation, the level and the type of award.
  4. Note where the certificate went and tick photo taken once there is a copy on your phone or computer.
  5. Go back over the record at the end of each school year with your child, and move finished goals down into the list.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

What counts as an achievement?

Whatever your family wants to remember: graded exams, swimming stages, club badges, school awards, and home firsts such as riding a bike without help or cooking a meal alone.

Why record where the certificate is kept?

Certificates get asked for years later, for a club, a course or an application. Knowing it is in the blue folder in the loft saves an afternoon of searching.

Why tick photo taken?

A photo or scan is a back-up if the paper is lost or damaged. The tick shows at a glance which certificates still need copying.

One record per child or one for the family?

One per child works best, so the list becomes that child's own record. Print a copy for each, or keep a separate online copy for each, saved in your own browser.
