Fillable PDF type and save

Spreadsheet formulas included

The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.

Fill it in online

The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.

Who this is for

Follow every credit report dispute to an answer. Each disputed entry gets a line for who it went to, when, the proof of sending and the evidence enclosed, the reply date you are working to and the outcome, and the sheet counts the days each one has waited.

Anyone who has found a mistake on a credit report and written to the agency or the lender about it, especially with several disputes running with different organisations at once.

What this helps you track

  • Every disputed entry and who the dispute was sent to
  • The date sent and the reference that proves it was sent
  • The evidence enclosed with each letter or form
  • The reply date you are working to, and how long each has waited
  • The outcome of each dispute, including any statement you added

How to use it

  1. Write your name and the agencies and lenders involved at the top.
  2. Give each dispute a line on the day you send it: the entry and what is wrong, who it went to, the date and the proof of sending reference.
  3. List the evidence you enclosed, and keep copies of everything you sent with the sheet.
  4. Enter the reply date you are working to, either one the organisation gave you or one you set yourself; response times differ, so check the rules where you are.
  5. When a reply arrives, enter the date and the outcome; days waited and the summary are worked out in the browser and the workbook.
  6. If an entry stays and you disagree, record whether you added a statement to your file, and keep the reply.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser
  • Excel / Google Sheets workbook

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Who should I send the dispute to?

You can raise it with the credit reference agency, with the lender or organisation that supplied the information, or with both. Many people write to both so the record is put right at the source. Give each its own line.

How long should I wait for a reply?

That depends on the rules where you are and on the organisation's own terms, so the sheet does not set a period. Write the date you were given, or one you set yourself, in the reply by column.

What counts as proof of sending?

A tracked or signed-for postage receipt, the reference from an online dispute form, or a saved copy of a dated email. Write the reference against the entry so it can be found quickly if anyone asks.

What does days waited show?

For a dispute with no reply yet, the days from the date sent to today. Once you enter the reply date it shows how long the reply took, so the disputes still counting up are the open ones.

What if the entry stays on my file?

Ask the organisation why and keep its reply. Where it is offered, you may be able to add a short statement to your file. A free advice service or the financial complaints route where you live can help with the next step.
