A diary of what you ate and how you felt, side by side. Each line records the date and time, what you ate or drank, where you were and who you were with, and how you felt before and after in your own words — with no calories, no rules and no reading of what any of it means.
Anyone who wants to notice the link, if there is one, between what they eat and how they feel; people who have been asked by a clinician, dietitian or counsellor to keep a diary; and anyone curious about a pattern they suspect but have never written down.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
No. There is no calorie column, no portion guide, no food list and no rule about what or when to eat anywhere on the sheet. It records what you write and nothing else.
No. It has no interpretation on it at all. If a pattern on the page worries you, that is a conversation for a doctor, dietitian or other professional, and the diary is a useful thing to bring.
Ask them, and use their instructions if they gave any. Some will want more detail, some less, and some have their own form. Where their instructions differ from these, theirs win.
No. Some people keep it as a food diary only and leave them blank; others find the feeling columns the useful part. Fill in what you want recorded.
Enough that you would recognise the meal a month later. Brand names, weights and exact amounts are optional unless somebody asked you for them.

When you went to bed, put the light out, woke and got up, night by night.
Every drink with its time, so the shape of a day shows up.
What happened and when, written close to the time rather than remembered later.
The areas of your life you decide to look at, in your own list.

Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
