Who this is for

The local services a new home needs. It lists the doctor, dentist, school, council and the rest down one side, with room for each one's name, address, phone number, what they need from you to register and the reference you are given.

Anyone who has just moved to a new town or district, families registering children at new schools and surgeries, and people who keep putting off the phone calls.

What this helps you track

  • Which local services you still need to find or register with
  • The name, address and phone number of each one you choose
  • What each service asks for before it will register you
  • The date you registered and the reference or number given
  • Local facts worth keeping to hand, such as bin days and the nearest station

How to use it

  1. Write the new address, the council area and the date you moved in.
  2. Work down the services listed, adding the name, address and phone number of the one you will use; add rows for anything else your household needs.
  3. Note what each asks for to register, such as proof of address or previous records, so you can gather it in one go.
  4. Write the date you registered and any reference, patient number or account number you are given.
  5. Fill the local notes box as you learn the area, and keep the sheet with your other household papers or save it in your own browser.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Which services should I register with first?

That depends on your household. People with ongoing prescriptions, school-age children or pets usually start there. The sheet lists the common ones so none is missed; the order is yours to decide.

What do services ask for when registering?

It varies by service and by area, which is why there is a column to write it down as you find out. Proof of address and identity are common requests; check with each service directly.

Is the electoral roll or council tax automatic when I move?

Not necessarily, and the rules differ from place to place. The sheet gives them a line each so you can find out from your council what applies and record when it was done.

How is this different from a change of address register?

The change of address register is for telling existing accounts you have moved. This sheet is for the new local services you have to find and join because of where you now live.
