Who this is for

A record of who has been told. Each organisation gets a line with the account reference, how you told them, the date and the confirmation you received, so nothing is left assumed.

Anyone moving house who wants proof that banks, suppliers, government bodies and everyone else have the new address, rather than a list of names crossed through.

What this helps you track

  • Every organisation that holds your address, grouped by type
  • The account or policy reference each one knows you by
  • How and when each was told
  • The confirmation reference and who gave it
  • Which ones still need chasing

How to use it

  1. Write the old address, the new address and the date the change takes effect.
  2. List the organisations section by section: finance, utilities, government, health, subscriptions and personal contacts.
  3. When you tell each one, fill in the method and the date; online forms and calls usually give a reference, so write it down there and then.
  4. Tick follow-up for any line where no confirmation arrived, and chase it once post starts arriving at the new address.
  5. Keep the register with your moving papers, printed or in the browser version, which saves in your own browser.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Is a tick not enough?

A tick shows you meant to tell them. A confirmation reference and a name show they received it, which matters if post keeps going to the old address or a statement goes missing.

Which organisations do people forget?

Often the ones contacted once a year: pension providers, old savings accounts, insurers on annual renewal, professional bodies, the optician and the dentist. Going through your bank statements for a year turns up most of them.

Does this tell me who I have to notify by law?

No. That differs by country and situation, so check the rules where you are. The register only records who you told and what they confirmed.

How does this differ from the change of address checklist?

The checklist is a list of who to tell. This register is the record of what happened when you did: method, date, reference and whether it needs chasing.
