Who this is for

A week laid out for a person and the people who support them. Each day is a row and each part of the day a pair of columns: what is happening, and what support is needed for it — so the person, the family and the staff all look at the same week and nobody turns up for a thing that was moved.

People in supported living and the staff who work with them, families planning a week around appointments, work, activities and visits, shared houses where several people's weeks have to fit together, and anyone whose support hours are planned around what they want to do.

What this helps you track

  • What is happening each morning, afternoon and evening across the week
  • What support each activity needs, written beside it
  • Appointments, work, activities and visits on one page, in the person's own plan
  • Who is coming and when, if you use the support column for names
  • A week the person can read, hold and change

How to use it

  1. Write the person's name, the week beginning and who the plan was made with at the top.
  2. Go through the week with the person, day by day, and write what they want or need to do in the activity columns.
  3. Beside each activity write the support it needs: a name, a lift, a prompt, nothing.
  4. Leave blank the parts of the day that are the person's own; a blank on this planner is not a gap to be filled.
  5. Print it and put it up where the person can see it, and give a copy to whoever is supporting them that week.
  6. Start a fresh one each week, or fill it in online and save it in your own browser.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Does the planner say how much support a person should have?

No. It has no hours, no ratios and no view about what anyone needs. The support column is for what has been agreed for that activity, written by the people who agreed it.

Who fills it in?

The person, wherever possible, with whoever supports them. It is their week. A planner filled in about somebody rather than with them tends to describe the staff's week instead.

What if plans change mid-week?

Cross it out and write the new one; the paper copy is meant to be scribbled on. Tell whoever was coming, and note it at the bottom so the next week's plan starts from what actually happened.

Can it be used for a shared house?

One sheet per person, side by side. Shared activities go on each sheet with the same support name, which is how clashes show up before the day.

Is this the same as the carer rota?

The rota is the staff's document: who is working when. This is the person's: what they are doing and what help each thing needs. The two are meant to be read together.
