The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.

Who this is for

A tracker for repeat prescriptions and the ordering round they need. Each row follows one item: when it was last ordered, how and where, when it was collected, the date you have decided to order again and what it cost you, if anything.

Anyone on several repeats, people ordering on behalf of a parent or partner, and households where a box running out on a Friday afternoon is a familiar problem.

What this helps you track

  • Each repeat item with the date it was last ordered and collected
  • How and where it was ordered, when several routes are in use
  • The date you have set to order next, and how many days that is away
  • Any charge you paid, added up over the sheet
  • Items that were not in stock or came back short, noted at the time

How to use it

  1. Write your details, your practice and your pharmacy at the top.
  2. Give each repeat item its own row and keep it there; update the dates rather than starting a new line each month.
  3. Record the order date and how you ordered — app, form, phone, in person — because the route changes how long it takes.
  4. Fill in the collection date when the item is actually in your hands, not when you were told it was ready.
  5. Enter your own next order date. The browser and spreadsheet versions count the days to it and total any charges; on paper, count them yourself.
  6. Use the notes column for anything that went wrong, so a pattern with one item or one route becomes visible.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser
  • Excel / Google Sheets workbook

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Does the tracker know when my repeat is due?

No. The next order date is yours to enter, based on what you have and what you were told. The sheet only counts the days between today and the date you typed in.

Why track how I ordered it?

Because the same item ordered through an app, a paper slip and a phone call can take very different lengths of time. A few months of rows makes that obvious, and the conversation with the practice easier.

What is the charge column for?

Whatever you actually paid, where you pay anything. Charges differ from country to country and person to person, so the sheet supplies no figure and simply totals what you enter.

How is this different from the medication record?

The medication record is the list of what you take, with the labels copied out. This tracker is the ordering round over months — what was ordered when, what was collected and what it cost.
